What are the Treatment options for Varicose Veins?
The progression of varicose vein symptoms can be stopped by eliminating the abnormal veins containing the faulty valves which are the source of varicose vein disease.
Compression Hose: is the most conservative option. Compression works by favoring the normal flow of blood from the superficial venous system to the deep venous system and upward to the heart. Compression decreases the amount of blood that is flowing abnormally backwards (refluxing) toward the feet because of incompetent valves in the leg veins. The downside of compression hose is that they must be worn continuously to be effective and that they are only a symptomatic treatment. Compression hose do not address the underlying cause of the problem which is the faulty valves in the leg veins.
Sclerotherapy: involves the injection of an irritating substance into an abnormal vein to destroy it. The body then re-routes the blood thru normal veins back to the heart. We mix tetradecyl sulfate and glycerine together to create a highly effective sclerosing agent. We use glycerine alone in dark skinned patients to decrease the chance of skin hyperpigmentation following sclerotherapy. Sclerotherapy can be used to destroy large or small veins by varying the concentration of sclerosing agent used. Spider veins are injected with sclerosing agent using a very small 30 guage needle. The patients are then fitted with a compression hose which they wear for 5 days. The compression hose prolongs contact of the sclerosing agent with the spider veins, helps to control bruising, and promotes the flow of blood upward to the heart. Compression is an essential component of effective sclerotherapy. Ultrasound guidance is used to precisely inject the sclerosing agent when performing sclerotherapy on larger veins. Larger veins require a higher concentration of sclerosant to close them. Therefore, precise placement using ultrasound guidance is required to prevent damage to surrounding tissues. Compression hose are worn for 2 weeks following sclerotherapy of larger veins.
Micro-Phlebectomy: involves the surgical removal of varicose veins through micro incisions made in the skin. Patients are comfortably sedated for the procedure and local anesthetics are used at the incision sites. The incisions are closed using steri-strips and patients wear compression hose for 1 week following the procedure. This procedure is often combined with endovenous laser ablation to fully treat severe varicose vein disease.
Endovenous Laser Ablation: involves the use of laser energy to destroy the underlying source of varicose veins. The laser fiber is placed within the abnormal vein using ultrasound guidance and laser energy is used to destroy the vein that is causing the varicose veins. The body then re-routes the blood through normal veins. Endovenous laser ablation has essentially replaced "vein stripping" as the definitive treatment of varicose vein disease. Patients are comfortably sedated for the procedure which typically takes less than one hour. Compression hose are worn for one week following the procedure. Patients are able to return to work the next day.
